Payer Resources
Most elective procedures require prior authorization. If a request is denied, physicians and patients have the right to appeal. We provide Lymphovenous Bypass (LVB) Prior Authorization and Appeal Letter templates to support your submissions. These templates and a full LVB Payer Toolkit are available upon request.
